What information is protected under the HIPAA Privacy Rule?

The HIPAA Privacy Rule protects all 'individually identifiable health information' that a covered entity or its business associate holds or transmits, in any format — electronic, paper-based, or spoken. This information is referred to as 'protected health information' (PHI)
