How can individuals file a complaint if they believe their privacy rights have been violated?

Individuals suspecting a breach of their privacy rights can lodge a complaint with the Office for Civil Rights (OCR) within the U.S. Department of Health and Human Services. Complaints must be filed within 180 days of the alleged violation, although an extension may be granted for good cause.
